Subject: Formal Response – Failure to Investigate & Provision of Incorrect Information

Dear PHSO

I write to you having received a service with a non functional customer survey. 

I therefore take this time to ask you why you failed to investigate the matter placed before my MP in her term with professionalism and or legal position as it seems your legal position was in direct rejection of the uk law  Further, you assert that a judicial review is the correct mechanism to challenge your decision. However, it is your organisation’s actions—including the mishandling of data and failure to adhere to due process—that have created this situation, causing harm to my family. I have also raised specific complaints regarding the conduct of your officers, yet at no point have I been referred to your HR department or advised on escalation to the ICO.–
